Community Services

The Community Services department empowers low-income individuals and families to become economically stable by providing emergency financial assistance and case management services. 

Emergency services may include one-time assistance in the form of rental or utility assistance payments.

The Case Management program empowers individuals and families to meet their educational and employment goals to obtain economic stability by transitioning them out of poverty. Our case managers complete a detailed household assessment to ensure the best support services are provided. Eligible individuals receive a combination of financial assistance and supportive services to meet their goals.

What information will I need to provide?

To complete your application, you will need to complete an intake interview with a staff member or apply online. After you complete the online application or intake interview, you will need to submit the following information:

Completed Client Consent Form –  required for all applications

Please have documentation of assistance needed available to provide to staff in person or by mail/email/fax. This documentation may include:

  • All pages (front and back) of a utility bill(s), if requesting utility assistance
    Invoice and lease, if requesting rental assistance
  • Invoice and lease, if requesting rental assistance

Proof of income for all household members who are 18 years of age or older for the thirty (30) days prior to the date the consent form is signed. This may include:

Other important information:

Clients eligible for Case Management may receive:

  • financial assistance to pay for tuition, books, exam fees, or other education-related costs;
  • assistance with job search and employability skills;
  • financial assistance to pay for uniforms, pre-employment physicals, or other job-related costs;
  • financial education in the area of budgeting, savings, and credit;
  • rental assistance on an as-needed basis or may apply for our Tenant-Based Rental Assistance program for up to two years of assistance;
  • financial assistance to pay utility costs on an as-needed basis; and,
  • financial assistance in the form of transportation vouchers or gas cards.

How Do I Apply?

Acceptable Documents

  • Four (4) current check stubs, if paid weekly
  • Two (2) current check stubs, if paid bi-weekly or twice a month
  • One (1) current check stub, if paid monthly
  • Social Security award letter
  • Retirement income letter
  • VA award letter
  • TANF letter or printout (for the previous thirty days)
  • Declaration of Income Statement, for adults who have not received any income
  • Employment Verification Form, for adults who do not have all paystubs
  • Documentation of any other income received by household members for the thirty (30) days prior to the consent form date
